    Soccer signups

    The Sunnyvale AYSO soccer organization will hold signups for youngsters ages 4-18 interested in playing in the 1999 fall season.

    Signups are scheduled for April 24, noon-5 p.m., at the Ortega Park Recreation Building; May 5, 7-9 p.m., at the Lakewood Park Recreation Building; and May 22, noon-5 p.m., at the Town Center Courtyard.

    Players must provide the names and phone numbers of a family doctor and emergency contact person; name of health insurance carrier; a passport-sized photo; and proof of age.

    Youth Soccer

    The De Anza Youth Soccer League will hold signups for boys and girls ages 4-18 interested in playing in the fall, on April 24 and May 15, 10 a.m.-2 p.m., at the Creekside Soccer Complex, Creekside and Miller in Cupertino.

    Youngsters must provide a birth certificate, a small picture of themselves, their doctor's name and phone number, and medical insurance. The registration fee is $85 for under-6 to under-8, $95 for under-10 to under-12 and $100 for under-13 and above. Scholarships are available.

    Registration applications are also available from the De Anza League Registrar, 7409 Fallenleaf Lane, Cupertino.
